Myopia is highly prevalent in the general population, affecting approximately 25%. There a few specifik risk and chalenges in patients with hight myopia during the cataract surgeru, but, two of the most commonly discussed cataract surgery risks for highly myopic patients are increased risk of retinal detachment and variable postoperative refractive error.
Discusions: Patient expectations is alwaus important, spetialy in hight myopic patients with cataract. It is important to have a thorough discussion with the highly myopic patient about setting realistic goals and expectations regarding cataract surgery outcomes.
One of the difficulties with preoperative calculations in highly myopic patients is the determination of axial length. However, almost all eyes with pathologic myopia are thought to have some degree of posterior staphylomata.There is also controversy over which formula is the best for calculating IOL power.
There are some procedures that we have to do to prevet some complications : ensuring adequate pressurization of the anterior chamber during capsulorrhexis, minimizing incision leakage and repeated collapse of the anterior chamber , decreasing irrigating bottle height and increasing flow… Cataract surgery in myopic eyes requires extra attention.
Concludions: It is important to have a diteild discussion with the highly myopic patient about setting realistic goals and expectations regarding cataract surgery outcomes. One of moust important rule is when possible, it is beter to place an IOL rather than leave a highly myopic patient aphakic. Although the patiens are often highly satisfied, cataract surgery in myopic eyes poses particular chalenges and risks.
