DOES THE STRESSFUL EVENTS DURING PREGNANCY INCREASE THE LIKELIHOOD OF PRETERM BIRTH ?

Systematic reviews on the relationship between preterm birth and different types of stress concluded that stressful events like major disasters and anxiety during pregnancy increase the risk for preterm birth
Aim: To determine the impact of the stressful events during pregnancy on the development of preterm birth in the Albanian context .
Material and methods: A case control study was carried out in the University Hospitals of Obstetrics and Gynecology during the 2012 in Tirana. The case group consists of 200 women who had had preterm delivery (before 37 week of gestation) and the control group consists of 400 women who had had a delivery in term. All women were asked if they have been under a stressful event during pregnancy. The logistic regression was used to measure the impact of having a stressful even during pregnancy on the development of preterm birth. A p < 0.05 is considered significant.
Results: The results indicated a significant higher percentage of stressful events during pregnancy among women with preterm delivery comparing to percentage of presence of stressful events among the women with delivery in term (33 % vs 11%; p<0.05). The crude odd ratio for the development of preterm birth due to the presence of the stressful events is 4.6 with 95 % CI 2-21.5, p<0.05. This odd ratio remained high and significant even after adjustment for social economic determinant 5.2 with 95 % CI 2.2-23, p<0.05.
Conclusion: The presence of the stressful events during the pregnancy increase the likelihood of development of preterm birth and thus it should be considered as a risk factor for the development of the preterm birth
