Diplopia as the first sign of meningeoma in the sella turcica space and posterior cranii cavity

Background: Serious approach by the ophthalmologist, in case of patient problems that seems insignificant and on time diagnosis, can contribute to the early detection of tumor processes in the brain as well as their adequate treatment.
Aims: To show how paresthesia in the back of the head and diplopia occurrence can indicate tumor process in the brain.
Patients and methods: The female patients came for the first time at ophthalmologist office with paresthesia in the back of the head and diplopia when looking into near and far objects. Ophthalmologist performed examination of eye fundus, diplopia tests according to Hess Lancaster computerized vision field, than referred the patient on CT of the cranii, brain MRI and neurosurgeon examination.i
Results: In findings we had changes in form of paresis of n. abducens lat.sin, and in vision field development of homonyme bitemporale hemianopsie. Status of the examination fundus was normal. MRI of the brain showed tumor mass in the left parasellar space (sinus cavernosus) which was spreading into posterior cranii cavity towards medulla spinalis. Tumor compressed VI brain nerve in the Dorello space which caused diplopia. At the clinic for neurosurgery patient underwent surgery of the meningeoma in the left pterocival region, and afterwards patient was treated with the Gamma Knife radiotherapy.
After conducted physical rehabilitation there was a stabilization of general state with significant reduction of diplopia, normal finding of eye fundus and preserved vision functions.
Conclusion: Successful interdisciplinary cooperation in diagnosis, therapy and rehabilitation brought healing to the patient.
