IMPACT OF SMOKING PRIOR TO AND DURING PREGNANCY ON THE DEVELOPMENT OF THE PRETERM BIRTH

Of all the life style risk factors, smoking has strong correlation with preterm birth and the risk is increased in correlation with the amount that the mother used a prior and during pregnancy.
Aim: To determine the impact of the smoking a prior to and during pregnancy on the development of preterm birth in the Albanian context
Material and methods: During the 2012, a case control study was carried out in the University Hospitals of Obstetrics and Gynecology in Tirana. The case group consists of 200 women who had had preterm delivery (before 37 week of gestation) and the control group consists of 400 women who had had a delivery in term. All women were asked if they have used to smoke prior to pregnancy. The logistic regression was used to measure the impact of smoking a prior to and during pregnancy on the development of preterm birth . A p < 0.05 is considered significant.
Results: All the women who used to smoke prior to pregnancy reported to quit smoking during pregnancy. The results indicated a significant higher percentage smoking a prior pregnancy among women with preterm delivery comparing to this percentage among the women with delivery in term (19 % vs 11%; p<0.05). The crude odd ratio for the development of preterm birth due to the smoking prior to pregnancy is 1.7 with 95 % CI 1.1-2.3 p<0.05. But this odd ratio is insignificant statistically after adjustment for social economic determinants OR = 1.7 with 95 % CI 0.9 -4.2, p<0.05 which mean that the smoking prior to is not an independent factor on development of the preterm birth
Conclusion: Even the smoking prior to pregnancy should not increase the likelihood of development of preterm birth , it still should considered a risky behavior for well being of the fetus.
